Advanced Practice Pharmacist, Oncology Pharmacotherapy Clinic

Responsibilities

• Initiate, modify and discontinue medications within a Collaborative Practice Agreement as defined by the California State Board of Pharmacy.
• Manage complex disease states to help patients reach optimal therapeutic goals.
• Simplify patient medication regimens, minimize the risk of harmful medication interactions and educate patients to enhance adherence and clinical outcomes.
• Develop and implement policies and procedures for continuous process improvement.
• Provide continuing education to clinical and medical staff.
• Precept Pharmacy APPE students and residents.
• Perform patient care assessments, order and interpret laboratory tests, prescribe, modify and discontinue medication therapies.
• Maintain clinical outcomes, communicate with medical providers, perform electronic documentation and refer patients to other health care providers.

Qualifications

• Advanced Practice Pharmacist (APP) licensure, DEA license, and board certification required.
• Residency training and oncology experience strongly preferred.

• Exceptional communication with all healthcare providers.
• Interviewing patients, family members, medical providers, and pharmacy staff to facilitate medication reconciliation.
• Collaborations and oversight of other pharmacy staff to maintain an environment of medication safety.
• Accuracy, comfort, and skill in managing chemotherapy plan templates within an EMR.
• Serving as a pharmacy and medication management resource to oncology medical and clinical staff.
• Collaborating with oncology leadership, medical staff, and pharmacy leadership to maintain and facilitate financial stewardship and clinical excellence/care.
• Serving as a pharmacy representative for regulatory compliance surveys for inquiries dealing specifically with the verification, preparation, dispensing, storage, waste, and administration of chemotherapeutic or hazardous medications.

Pay

The compensation range is $91.28 to $127.78 per hour. The base pay is only one component of Sutter Health's comprehensive total rewards program. Eligible positions also include a comprehensive benefits package.
