About the role

The Advanced Practice Nurse will conduct re-certification visits to patients on hospice in accordance with local, state, and federal regulations and collaborate with the interdisciplinary team to ensure high-quality palliative care.

Responsibilities

• Conduct re-certification visits to patients on hospice in accordance with local, state, and federal regulations.
• Communicate visit findings to designated members of the interdisciplinary team, including the Medical Director.
• Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, on-going aggressive treatment).
• Consult with the referring physician, the acute care team, and other members of the interdisciplinary team to contribute to the patient's plan of care.
• Prepare and maintain accurate patient records, charts, and documents.
• Assist in the development of clinical practice guidelines/standards in support of quality palliative care as needed.
• Ensure collaboration with patients, caregivers, medical directors, and members of the IDT.

Requirements

• Master's Degree in Nursing.
• Nurse Practitioner's license.
• 2-4 years of experience working within hospice or a related field.
• Valid driver's license and reliable transportation.
• Willingness to travel.
