Advanced Practice Nurse (PRN)
About the role
The Advanced Practice Nurse will be responsible for conducting re-certification visits to patients on hospice in accordance with local, state and federal regulations. Communicate visit findings to designated members of the interdisciplinary team, including the Medical Director. Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, on-going aggressive treatment). Consult with the referring physician, the acute care team, and other members of the interdisciplinary team to contribute to the patient's plan of care. Prepare and maintain accurate patient records, charts and documents. Assist in developing clinical practice guidelines/standards in support of quality palliative care as needed. Ensure collaboration with patients, caregivers, medical directors and members of IDT.

Requirements
- Must have a Nurse Practitioner's license.
- Must have 2-4 years experience working within hospice or related field.
- Must have a Master's Degree.
- Must have a valid driver's license and a reliable source of transportation.
- Willingness to travel.

- Master's Degree in Nursing.
