About the role

Join our team in Plymouth, MA as an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) and take on comprehensive, longitudinal clinical and administrative coordination for a specific panel of patients. You will perform examinations, diagnoses, and treatments in a primary care setting, overseeing care plans, consultations, referrals, and communications with specialists. You’ll manage patients with acute and chronic illnesses, adjust treatment regimens, and ensure ongoing patient management. Additionally, you’ll partner with patients and families, provide consultation for hospitalized patients, and support staff to ensure departmental effectiveness and efficiency.

Responsibilities

• Provide comprehensive clinical and administrative coordination for a panel of patients.
• Perform examinations, diagnoses, and treatments in a primary care setting.
• Supervise care plans, including consultations, referrals, and communications with specialists.
• Monitor and manage patients with acute and chronic illnesses, adjusting treatment regimens as needed.
• Serve as the care plan author and supervisor.
• Partner with patients and families to manage care plans.
• Provide consultation for hospitalized patients within your panel.
• Support and direct staff to ensure departmental effectiveness and efficiency.

Requirements

• Graduate of an Accredited Nurse Practitioner or Physician Assistant Program.
• Active, unrestricted Massachusetts NP or PA license.
• National Board Certification.
• Current active Mass DPH Controlled Substances Registration or ability to obtain post-hire.
• Current active DEA licensure/prescriptive authority or ability to obtain post-hire.
• Maintains BLS certification and professional certification as appropriate.

Preferred Qualifications

• 2+ years of Advanced Practice Clinician (APC) experience.

Pay

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on factors such as local labor markets, education, and work experience. Compensation may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
