Responsibilities

• Manage your patient panel and cultivate long‑term patient relationships.
• Provide comprehensive, compassionate care in a team‑based, patient‑centered medical home.
• Work with underserved populations across rural, coastal, valley, and metro Oregon communities.
• Utilize EPIC EMR system with support from certified bilingual medical assistants.

Qualifications

• Master’s or Doctoral degree – Nurse Practitioner, OR
• Master’s degree – Physician Assistant, OR
• Bachelor’s degree in Health Science plus a two‑year Physician Assistant training program.
