About the Role
We are seeking a quality-driven nurse practitioner or physician assistant to join our post-acute care team in Manchester, Connecticut. This full-time opportunity involves rounding in skilled nursing facilities with excellent support, training, autonomy, and earning potential. Compensation is fee-for-service with an estimated annual range of $142,000 to $168,000 and no cap on productivity income potential.
You will provide high-quality care to a diverse patient population, collaborate with an experienced multidisciplinary team, and build meaningful relationships with patients and their families. Our comprehensive clinical and operational leadership team will support your transition with a 3-month onboarding program, a dedicated clinical field mentor, and access to industry-leading experts in post-acute medicine. The practice is part of TeamACO, a dedicated long-term Accountable Care Organization participating in the Medicare Shared Savings Program, which may result in shared savings if certain criteria are met. Our Medical Director Leadership Academy (MDLA) offers best-in-class training to position you for success in quality improvement initiatives and collaboration with facility leadership.
Responsibilities
- Perform comprehensive patient assessments, including medical histories and physical exams
- Develop, implement, and adjust individualized care plans based on patient needs
- Monitor progress and coordinate care to optimize outcomes
- Collaborate closely with physicians, nursing staff, and other healthcare professionals in a team approach
- Educate patients and families on health management and preventive strategies
- Maintain accurate, timely medical records in compliance with regulations
Requirements
- NP or PA license (State of Connecticut) and DEA
- Experience in post-acute, acute, ED, or clinic settings preferred but not required; new graduates welcome
- Strong clinical foundation and knowledge of healthcare regulatory standards
- Excellent communication and interpersonal skills
- Collaborative, team-oriented approach with a positive outlook
- Ability to commute/relocate to Meriden, CT (required)
