About the Role

Provides clinical leadership to enhance quality and cost compliance, managing inter-professional teams and delivering advanced nursing care within regulatory standards. Acts as a key resource for clinical and operational guidance, ensuring optimal patient outcomes. Works under the supervision of the Clinical Director for the Nurse Practitioner Program.

Responsibilities

  • Manages full scope of advanced nursing practice for targeted populations, evaluating responses and updating care plans accordingly.
  • Develops and trains staff on clinical protocols in collaboration with physicians.
  • Provides comprehensive care, including physical exams, interventions, treatment ordering, health maintenance, and patient education.
  • Collaborates with patients, families, and healthcare teams for assessment and care planning across settings.
  • Ensures clinical services meet standards such as Patient-Centered Medical Home, meaningful use, and quality measures like HEDIS and QARR.

Qualifications & Skills

  • Licensed Registered Nurse and Nurse Practitioner in New York State, with certifications in adult, family, or gerontology health.
  • Current ANCC or AANP certification; psychiatric NPs require PMHNP-BC certification.
  • Master’s degree in Nursing from an accredited program; preferred experience includes two years as a nurse practitioner, home care, or managerial roles.
  • Knowledge of quality measures, ICD-10, and CPT coding; bilingual skills may be required.
