About the role

As an Advanced Practice Clinician (APC) at knownwell, you will work alongside a team of committed physicians, APCs, registered dietitians, registered nurses, and registered pharmacists to provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity-related conditions in a primary care setting. This position offers an opportunity to make a significant impact on patients' lives by helping them improve their overall health.

Responsibilities

  • Practice exceptional medicine within knownwell, seeing adults for primary care with a focus on metabolic health or metabolic health only.
  • Share a patient panel with MD/DO and other APCs.
  • Embody the knownwell mission and values of patient inclusivity in care delivery.
  • Coordinate with the broader care team to ensure patients receive necessary support.
  • Document accurately for billing, including review of remote patient monitoring data.
  • Communicate with patients in a timely manner through our portal for pre-screened clinical concerns.
  • See adults ages 16+ for metabolic health and primary care, ensuring care is accessible regardless of health status or comorbidities.
  • Tailor care to each patient’s unique health journey and aspirations.
  • Provide a judgment-free, comfortable environment for patients.
  • Offer encouragement as patients work toward their goals.
  • Escalate concerns to the site leadership team.
  • Provide clinical guidance to medical assistants and nursing teams as assigned.
  • Maintain current licensure and certifications.

Requirements

  • Master's degree (MSN) and certification by ANCC or AANPCB as a family nurse practitioner, or Master's degree in Physician Assistant studies and certification by NCCPA.
  • Unrestricted DEA licensure and prescribe authority (per state practice regulation).
  • At least 2 years of clinical experience in an ambulatory care setting, family or internal medicine; experience in chronic condition management and weight management preferred.
  • If no formal training in weight management, willingness to participate in knownwell’s clinical education and training plan or pursue OMA APC certification (Obesity Medicine Association).
  • Active, unrestricted license to practice in NY, or willingness to obtain.
  • Willingness to obtain additional licensures and pursue independent practice if needed.
  • Excellent communication and interpersonal skills.
  • Dedication to staying current with developments in primary care and obesity medicine.
  • Commitment to inclusive, compassionate, and shared decision-making care.
  • High emotional intelligence and ability to make patients feel at ease without judgment.
  • Willingness to adopt new patient- and provider-oriented technologies.
  • Excitement to work collaboratively with physicians, APCs, registered dietitians, medical assistants, health coaches, and other staff.
  • Authorized to work in the United States (visa sponsorship not available).

Schedule

  • Monday–Friday with flexible scheduling.
  • Minimum 32 patient contact hours (+ 8 hours of administrative time).
  • Hybrid model of care (in-clinic and virtual).
  • No scheduled evening or weekend responsibilities outside of phone on-call.
  • Phone on-call rotation (frequency TBD); low call volume with 24/7/365 physician backup.

Location

Midtown Manhattan (EMR: Athena).
