About the role
Provides clinical leadership to ensure compliance with quality and care standards. Manages inter-professional teams, offering clinical and operational guidance. Delivers advanced nursing care in accordance with state, federal, and national standards, under supervision of the Clinical Director.
Responsibilities
- Administers full scope of advanced nursing practice for specific patient populations, including assessments, treatment plans, and interventions.
- Leads interdisciplinary care teams, participates in rounds, and manages patient care across settings.
- Performs post-discharge visits, comprehensive assessments, and urgent interventions, including medication management and patient education.
- Collaborates with patients, families, and healthcare providers to develop and implement care plans.
- Ensures compliance with care standards, manages documentation and billing, and maintains professional credentials.
- Provides care virtually, via telehealth, or in patients’ homes, adapting to various environments.
- Acts as a peer reviewer, preceptor for students, and participates in quality improvement initiatives.
- Assumes responsibility for ongoing professional development and participates in special projects as needed.
Qualifications
- Licensed Registered Nurse and Nurse Practitioner in the state, with certification in adult, family, or gerontology health.
- Master’s degree in Nursing from an accredited program.
- Minimum two years of NP experience preferred; clinical home care or managerial experience advantageous.
- Knowledge of quality measures, coding, and reimbursement practices required.
- Bilingual skills may be required based on operational needs.
