About the role

The Advanced Clinical Pharmacist is responsible for providing all levels of pharmacy clinical services, including an advanced level of consultative or direct pharmaceutical care as a member of the healthcare team. This role involves monitoring patient medication regimens, providing interventions, and supporting all aspects of pharmacy operations. The practitioner collaborates with physicians, nurses, and other health professionals to design, implement, monitor, and evaluate pharmacotherapy to ensure effective, safe, and economical patient care.

The Advanced Clinical Pharmacist assists the Director of Pharmacy and Clinical Manager in developing pharmacy clinical services and participates in the clinical training of pharmacists, pharmacy residents, students, and other health professionals. Practices follow the policies and procedures of Ocala Health System. May be required to work at Ocala Regional Medical Center, West Marion Community Hospital, or The Free Standing Emergency Departments as needed.

Responsibilities

• Proficient in all skills as per Clinical Pharmacist essential duties.
• Develop, promote, maintain, and evaluate patient care services to achieve appropriate care standards, quality improvement, and cost savings.
• Demonstrate competence in critical thinking, interpersonal relationships, and technical skills.
• Review non-formulary requests and provide overall formulary management for critical care and trauma areas.
• Attend meetings as required.
• Provide clinical pharmacy services to patients in the ICU and/or Trauma areas.
• Assist the Clinical Pharmacy Manager in developing the clinical goals of the Pharmacy department.
• Maintain an active role in Medication Utilization Evaluation and Adverse Drug Reaction reporting.
• Support staff in the Pharmacy department and provide help and assistance when possible.
• Keep fellow employees informed of relevant information and establish effective working relations with staff.
• Perform other related duties as assigned or requested by the Pharmacy leadership team.
• Interpret orders and transcribe or verify them accurately and efficiently into the pharmacy computer system.
• Accurately compound and dispense pharmaceuticals.
• Comply with facility policy and procedures and regulatory requirements.
• Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
• Provide clinical consultation and clarification to practitioners, suggesting appropriate, cost-effective therapeutic alternatives to medical staff as needed.
• Sustain the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges, and promoting rational drug therapy selection.
• Attend and actively participate in daily multidisciplinary and bedside rounds, as appropriate.
• Support the hospital formulary decision process by working with medical staff to promote rational drug therapy and make improvements identified by drug utilization evaluations.
• Provide accurate and timely medication information to healthcare staff and patients.
• Actively participate in pharmacy clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing, and other programs as defined by Pharmacy Leadership.
• Participate in the antimicrobial management and other clinical programs.
• Document all clinical activities and interventions accurately and completely.
• Responsible for mentoring and education as well as ongoing analysis of interventions to ensure accurate and appropriate documentation and provide summary reports (if assigned).
• Support and participate in the goals of the organization’s quality improvement, risk management, and patient safety program, including National Patient Safety Goals, Core Measures, and HCAHPS medication-related initiatives.
• Complete notification reports regarding medication errors and adverse drug reactions.
• Participate in the data collection of drug utilization evaluations, adverse drug reactions, and pharmacy quality initiatives.
• Participate in the training of new pharmacists, pharmacy students, residents, technicians, nurses, and physicians.
• Participate in preceptorship of pharmacy students and/or residents.
• Maintain professional competency sufficient to meet current and future needs of the facility.
• Apply knowledge of patient populations served in the care of those patients.
• Provide basic technical support for automated dispensing machines and other pharmacy computer systems.
• Effectively supervise pharmacy operation activities and provide support to the whole pharmacy team.
• Comply with the facility pharmacy schedule to work physically at patient care areas to interact with medical staff, nurses, and other members of the healthcare team.

Requirements

• Bachelor’s Degree in Pharmacy.
• 2 years of experience as a Pharmacist.
