Advanced Clinical Pharmacist
About the role
As an Advanced Clinical Pharmacist, you will be responsible for one or more specialty services within the hospital, ensuring the safe and effective pharmaceutical care for patients. You will act as a patient advocate, collaborating with healthcare professionals as a member of a multidisciplinary team. In addition, you will lead pharmacy services within the hospital by serving or chairing hospital committees, presenting at conferences, etc.
Responsibilities
- Prepare and dispense medications safely and accurately.
- Provide drug information to patients and families.
- Issue controlled substances to patient care areas and maintain records as required by law and corporate policies and procedures.
- Lead committees responsible for medication-related policies and procedures.
- Serve as a mentor to pharmacy students, residents, and colleagues in a manner that fosters highly professional pharmaceutical care and high personal standards of integrity.
- Work with physicians to monitor medication therapy for contraindications, interactions, allergies, and appropriate drug and dose.
Qualifications
- B.S. or Pharm D. degree in Pharmacy
- Required: Graduate of an accredited PGY-1 pharmacy residency program
- PREFERRED: Graduate of an accredited PGY-2 pharmacy residency program
- 5+ years of inpatient hospital experience
- 3+ years of HCA inpatient hospital experience
- Maintains current pharmacist licensure, with the state board of pharmacy, is required.
- Pharmacist Board Certification is required
- Specialty certifications, as determined by the facility needs, may be required.
