About the Role
This full‑time position is for a certified Nurse Practitioner or Physician Assistant who will serve as the primary clinical provider for MyCare patients. The role is 100% outpatient with the possibility of on‑call services.
Responsibilities
- Provide patient care Monday – Friday, 8 am – 5 pm with a rotating and flexible on‑call schedule.
- Utilize eClinical Works (eCW) and partnering programs to ensure ease of use for patients and the practice.
- Focus on continuity of care, health maintenance, and disease prevention, working with patients and families to prevent, diagnose, treat, and manage multiple comorbidities.
- Participate in integrated, multidisciplinary team‑based care to improve patient outcomes and meet patient preferences.
- Prioritize preventative care and proactive management of chronic conditions to improve outcomes, reduce patient costs, and increase practice revenue.
- Manage a primarily adult‑aged patient population.
Qualifications
- Certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC) program, focusing on adult medicine, **or** certification through an accredited PA program focusing on adult medicine.
- Current license as an Acute, Adult/Gerontological, or Family Nurse Practitioner in the state of practice, or ability to obtain one prior to employment.
- Minimum two years of experience as a Nurse Practitioner.
- Bilingual English‑Spanish preferred.
- Holds or is eligible for DEA licensure and CDS licensure (if required in the state of practice).
- Current CPR and Basic Cardiac Life Support certification.
Required Skills
- Strong organizational and prioritization abilities.
- Proficiency with Electronic Medical Record (EMR) systems.
- Two years of experience with Medical Advantage, HEIDS, MRA, and ICD‑10 coding.
- Comfort managing a primarily adult‑aged patient population.
- Demonstrated ability to problem‑solve complex, multifaceted situations.
Preferred Skills
- Bilingual (English‑Spanish) communication.
Pay
Base salary range: $130,000 – $135,000 (based on experience). Annual performance bonus up to 15% of base salary.
