Responsibilities

Clinical Care

• Provides assessment, triage, and therapeutic management of patient/family needs throughout the disease process as part of an interdisciplinary team in inpatient, ambulatory, or outpatient settings.
• Works collaboratively with attending/supervising physician(s) in accordance with state law, hospital regulations, policies, protocols, and clinical pathways, with potential for autonomous team-based care.
• Obtains medical histories, performs physical examinations, reviews/orders diagnostic/therapeutic tests and radiologic results, and formulates/manages patient treatment plans.
• Performs procedures, prescribes treatments and medications within institutional and state guidelines, and professional competency standards.
• May function as first or second assistant on operative cases, using fine motor skills and manual dexterity, if within scope and job prerequisites.
• Responds to emergency situations within YNHHS parameters.

Patient Education & Psychosocial Support

• Educates patients/families on care, treatment plans, side effects, and discharge plans in collaboration with the supervising/collaborating physician.
• Provides psychosocial support to patients, family members, and significant others as part of a multidisciplinary team.

Information Management

• Ensures appropriate documentation in accordance with hospital and regulatory requirements.
• Protects patient information from unauthorized access, modification, destruction, or disclosure.

Quality Management

• Demonstrates high standards of moral and ethical behavior, professionalism, compassion, and commitment to patient care.
• Ensures compliance with healthcare regulatory requirements, standards, and recommendations.
• Reports safety events and near misses, demonstrating high reliability organizational behaviors.

Professional Development

• Engages in projects to maintain and advance professional competency.
• Serves as a clinical resource for new house staff and provides service-specific orientation.
• Maintains all employee requirements as determined by hospital policy.
• May serve as a clinical preceptor for NP/PA students and other healthcare professionals, and mentor pre-PA/NP candidates.
• Collaborates on research, quality assurance, and performance improvement projects.
• Assists in forming protocols for clinical practice or further educational and clinical expertise.

Qualifications

• Graduate from an accredited Nurse Practitioner or Physician Assistant Program.
• Clinical working experience as an NP/PA in the specialty/service line is preferred but not required.
• APRNs, PAs, CRNAs, and CNMs must be appointed to the Affiliated Medical Staff of a YNHHS hospital and complete the Medical Staff credentialing process, including providing evidence of current competence and reference letters.

Licensure

• Active PA Certification by the National Commission of Certification of Physician Assistants (NCCPA) or Connecticut State Registered Nurse License and Connecticut State Nurse Practitioner License.
• Board specialty certification, as appropriate for the position.
• Connecticut State Physician Assistant License (if applicable).
• ACLS/BLS Certified, when indicated for the position profile.
• DEA License.
• Additional certifications may be required based on specialty (e.g., PALS for pediatric positions, NRP for neonatal care).
• Additional licensure, certifications, and/or continuing education credits may be required during employment.
