Responsibilities
- Provides comprehensive assessment, triage, and management of patient and family needs throughout the disease process as part of an interdisciplinary healthcare team.
- Works in inpatient, ambulatory, and outpatient settings, delivering high-quality patient care across diagnostic, treatment, and prognostic phases.
- Collaborates with supervising or collaborating physicians in accordance with state laws, hospital regulations, policies, clinical pathways, and protocols.
- May include autonomous, team-based care for designated patient populations.
- Obtains medical histories and performs physical examinations.
- Orders, reviews, and interprets diagnostic and therapeutic tests, including imaging.
- Develops and manages patient treatment plans.
- Prescribes medications and treatments consistent with institutional and state guidelines.
- Performs clinical procedures within professional scope and competency.
- In select specialties, assists as a first or second assistant in surgical procedures when educational and credentialing requirements are met.
- Responds to emergency situations in alignment with YNHHS policies.
- Educates patients and families about diagnoses, treatment plans, medications, side effects, and available resources.
- Collaborates with the supervising physician for admission instructions, ongoing care guidance, and discharge planning.
- Provides compassionate psychosocial support to patients, families, and significant others, maintaining ethical and professional standards of care.
- Ensures accurate, timely, and complete documentation in compliance with hospital, legal, and regulatory requirements.
- Safeguards patient information in all formats to prevent unauthorized access, alteration, or disclosure.
- Demonstrates professionalism, ethical conduct, compassion, and a strong commitment to patient care.
- Maintains compliance with all applicable healthcare regulations, standards, and best-practice recommendations while consistently promoting patient safety and quality outcomes.
- Engages in ongoing professional development to maintain and enhance clinical competency.
- Serves as a clinical resource and educator for new staff and trainees.
- Participates in service-specific orientation and educational initiatives.
- Reports safety events and near-misses to support high-reliability practices.
- Maintains all licensure, certification, and employee requirements.
- Precepts NP/PA students and mentors other healthcare professionals.
- Collaborates on research, quality improvement, performance improvement initiatives, and protocol development to advance clinical practice.
Qualifications
- Graduate from an accredited Nurse Practitioner or an accredited Physician Assistant Program.
- Experience as an NP/PA in the specialty/service line is preferred but not required.
- Active PA Certification by the National Commission of Certification of Physician Assistants (NCCPA), Connecticut State Physician Assistant License, ACLS/BLS Certified, when indicated for position profile DEA License.
- Pediatric position profile requires PALS, when indicated for neonatal care NRP Certification.
- Licenses, certifications, and/or continuing education credits may be required based on position and area of specialty.
