About the role
Apalachee Center, Inc. provides comprehensive behavioral health services across eight counties in North Florida (Franklin, Gadsden, Jefferson, Leon, Liberty, Madison, Taylor, and Wakulla). The purpose of mental health targeted case management is to assist individuals in gaining access to needed medical, social, educational, and other services. The primary goal is to optimize the functioning of recipients who have complex needs by coordinating quality treatment and support services in the most efficient and effective manner. Services and service frequency are tailored to each recipient’s individual needs, goals, and abilities.
Responsibilities
- Assessments: Conduct assessments of assigned clients, including Preliminary Bio-psychosocials, Psychosocial/Comprehensive Assessment, Medical Questionnaires, and related intake assessments within 30 days of intake. Contact family and other providers (with appropriate consent) to gather assessment information. Update assessments per Client Record Handbook guidelines, finalize documents in Avatar, and submit within 24 hours. Maintain 85% compliance with assessment requirements.
- Home Visits: Conduct home visits prior to completing the intake assessment to evaluate client safety and well-being in their residence, unless staffed with a supervisor and written justification is provided. Use a laptop as appropriate. Maintain 85% compliance with home visit requirements during the rating period.
- Client Service Plans: Develop individualized service plans in partnership with the client and their guardian (if applicable). Plans must include measurable goals, timeframes for achievement, and be signed by the client and guardian. Update service plans every 6 months or when significant changes occur (e.g., hospitalizations, goal achievement). Maintain 85% compliance with service plan requirements.
- Case Management: Assist clients in accessing financial, insurance, employment, medical, social, educational, and other services. Collaborate with the client and their natural support system to implement the service plan. Use the Level of Care system to monitor appropriate care levels. Follow up on services to assess effectiveness and community inclusion. Maintain 85% compliance with case management requirements.
- Service Provision: Meet a service productivity standard of at least 75% (6 out of 8 work hours) for case management services, documented per Client Record Handbook, Avatar requirements, and agency standards.
- Progress Notes: Document all services in the client record per Client Record Handbook guidelines. Finalize notes in Avatar within 24 hours of service delivery. Notes must detail how efforts align with the service plan and reflect client progress or lack thereof. Maintain 85% compliance with progress note requirements during the rating period.
- High-Risk Clients: Identify high-risk clients and complete suicide risk assessments during intake or when suicide ideation, plans, or attempts are noted. Review assessments with a supervisor or MD/ARNP and route forms appropriately. Document staffing with the treatment team. Include suicide risk as a problem statement with measurable objectives, monitoring schedules, and interventions in the Client Service Plan (CSP). Assist clients in completing a Personal Safety Plan. Maintain 85% compliance with suicide risk protocols.
