About the role
The purpose of mental health targeted case management services is to assist individuals (recipients) in gaining access to needed medical, social, educational, and other services. The primary goal is to optimize the functioning of recipients who have complex needs by coordinating the provision of quality treatment and support services in the most efficient and effective manner. Services and service frequency should accurately reflect the individual needs, goals, and abilities of each recipient.
Responsibilities
- Assessments: Conduct assessments of assigned clients, including Preliminary Bio-psychosocials, Psychosocials/Comprehensive Assessment, Medical Questionnaires, and related intake assessments within 30 days of intake. Contact family and other providers (with appropriate consent) to gather assessment information. Update required assessments per Client Record Handbook guidelines. Finalize all documents in Avatar and submit assessments within 24 hours. Maintain 85% compliance with assessment requirements.
- Home Visits: Conduct home visits prior to completing the intake assessment to evaluate client safety and well-being in their residence, unless staffed with a supervisor and written justification is provided. Use a laptop as appropriate. Meet at least 85% compliance with home visit requirements during the rating period.
- Client Service Plans: Develop individualized service plans in partnership with the client and their guardian (if applicable). Include measurable goals and objectives derived from the client’s assessment, with identified time frames for achievement. Update service plans at least every 6 months or when significant changes occur (e.g., hospitalizations, goal achievement). Maintain at least 85% compliance with Client Service Plan requirements.
- Case Management: Assist clients in accessing needed financial, insurance, employment, medical, social, educational, and other services. Work with the client and their natural support system to develop and implement the service plan. Use and maintain the Level of Care system to monitor appropriate care levels. Follow up on services to determine effectiveness and client progress. Maintain at least 85% compliance with case management service requirements.
- Service Provision: Meet a service productivity standard of at least 75% (or 6 out of 8 work hours) for providing Case Management services. Ensure compliance with clinical/quality criteria, supported by appropriate documentation in the Client Record Handbook, Avatar, and agency standards.
- Progress Notes: Document all services in the client record per Client Record Handbook Guidelines. Finalize notes in Avatar within 24 hours of service provision. Progress notes must detail how case management efforts align with the client’s service plan and reflect progress or lack thereof. Maintain at least 85% compliance with progress note requirements during the rating period.
- High Risk Clients: Identify high-risk clients and complete suicide risk assessments during intake or at any time during treatment if a client exhibits suicide ideation, plans, or attempts. Review risk assessments with a supervisor and/or MD/ARNP, and route forms to appropriate staff. Document staffing with the treatment team in an individual note. Include suicide risk as a problem statement with measurable objectives, monitoring schedules, and treatment interventions on the Client Service Plan. Assist clients in completing a Personal Safety Plan. Maintain at least 85% compliance with suicide risk requirements.
