Adult or Child Developmental Home Provider

About the role

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states. This position involves providing a 24/7 family-like setting for an individual with developmental and/or physical disabilities.

Responsibilities

• Ensure the use of best practices and technology to teach individuals how to acquire physical, intellectual, emotional, and social skills needed to live independently.

Requirements

• At least 21 years of age
• Pass a background check
• Obtain a Level One Fingerprint card
• Adequate transportation
• At least 1 year of previous work experience as a caregiver
• Willingness to complete 22 hours of training through Community Options (CDH Providers are required to complete an additional 15 hours of Foster Parent College training)
• Provide 3-character references
• Submit paystubs for at least 1 month
• Complete an annual physical
• Pass the State of Arizona Life Safety Inspection
• Complete the interview process with a licensing specialist
• Submit all required documents for the licensing process
• Be open to supervisory visits in your home, including unannounced visits

Benefits

This independently contracted position is paid at a daily habilitation rate, PLUS receives monthly room and board from the individual they support – this income is tax-exempt.
