Adult In-Home Case Manager — Escambia County-Atmore and Brewton

Location: One position in Atmore and one position in Brewton, Alabama Description: Adult In-Home Case Manager Salary: $17.31 hr Qualifications: Bachelor degree required Preferred Experience: Bachelor degree required preferrably in a mental health related field. Work with individuals with severe mental illness. Duties and Responsibilities: Provide case management services to individuals with severe mental illness in their home or community setting as a member of a two-person team paired with a master level counselor. Complete needs assessment and case plan for consumers. Coordinate services and secure resources with other community agencies. Assist in securing Medicaid/Medicare/Social Security benefits, financial assistance, housing, food, and other health services.
