Pay
$17.31 per hour
Qualifications
- Bachelor degree required
- Bachelor degree preferably in a mental health related field (preferred)
Responsibilities
- Provide case management services to individuals with severe mental illness in their home or community setting as a member of a two-person team paired with a master-level counselor
- Complete needs assessment and case plan for consumers
- Coordinate services and secure resources with other community agencies
- Assist in securing Medicaid/Medicare/Social Security benefits, financial assistance, housing, food, and other health services
