In coordination with therapeutic staff, the incumbent will assist consumers by providing information, education, and training to build knowledge, develop skills, and change attitudes with a goal of increasing the consumer’s independence, productivity, self-determination, integration, and inclusion in the community. The majority of the Case Manager’s time will be spent providing services to consumers in consumer’s homes, vocational site, program site or other community settings, with minimal time spent at the Center. This position reports directly to the CSS Director or designee. Major Job Responsibilities Provide SOAR case management for clients seeking Social Security BenefitsProvide SOAR training when necessaryProvides case management to SPMI adults supporting consumer’s treatment plan goals and advocacy in the areas of: Education/Vocation, Residential, Financial, Health, Leisure, and Social Supports utilizing community resources on behalf of the consumer as neededCollaborates at the local and state level on behalf of ELC in coordinating services and resources for SPMI consumersAssists in collection of program data and provides reports as necessary to immediate supervisorWorks in conjunction with other mental health center staffParticipates in treatment planning based on best practice theories and applies interventionsMeets direct service expectationsMeets deadlines for daily paperwork and reportsProvides quality documentationProvides supportive intervention when needs arise outside of scheduled hoursCompletes other duties as directed Completes all required trainings as assigned Complies with all ELC policies and procedures ELC is an Equal Opportunity Employer (EOE) and considers all applications irrespective of race, marital status, sex, age, skin color, religion, nationality, veteran status, disability or any other characteristic protected by law.
