Admissions/Team Nurse

About the role

The Admissions/Team Nurse provides coverage for Nurse Case Managers to cover patient/staffing needs and processes admissions for eligible hospice patients in coordination with the Team Manager.

Schedule

Monday-Friday; 8:30am - 5:00pm

Location

Marin County with additional coverage to Sonoma County as needed

Essential Duties & Responsibilities

• Make in-person nursing visits to patients as assigned, or as indicated when case managing.
• Use the nursing process to assess, plan, intervene and evaluate following patient/family needs and goals.
• Provide effective management of symptoms, including prompt escalation to providers or others as indicated.
• Cookbook patient/family services and prioritization of needs with the members of the transdisciplinary team or outside providers/services.
• Make telephone contact and/or conduct assessment visits with potential hospice patients to provide information on agency services and assess patient status.

Education/Experience

• One year experience as a professional nurse in the past three years or Bachelor’s degree in nursing from an accredited School of Nursing.
• Experience in acute care/ home health and/or hospice setting preferred.

Certificates, Licenses, Registrations

• Current license as a registered professional nurse in the State of California.
• Public Health Nursing certification highly desirable.
• HPNA certification preferred.

Pay Range

$58.34 - $72.95 hourly
