About the role

The Afterhours Registered Nurse provides comprehensive hospice care outside regular hours, including nights and weekends. They respond to crisis calls, triage patient needs over the phone, and conduct initial evaluations to determine hospice eligibility within 48 hours of referral. The nurse performs afterhours visits such as admissions, death, recertification, and routine assessments, addressing physical, psychosocial, emotional, and environmental needs. Documentation of care, goals, and patient responses is essential. The role requires adaptability to diverse home situations and a commitment to patient-centered care.

Responsibilities

• Respond to afterhours crisis calls with urgency
• Perform timely in-person patient visits
• Evaluate and document patient needs and responses
• Assess patient eligibility for hospice services
• Provide care in varied home environments

Qualifications

• Current unrestricted RN licensure in the practice state
• Valid driver’s license, insurance, and reliable transportation or public transit access

Skills

• CPR certification (preferred)
• 1+ years clinical experience (preferred)
• Hospice care experience (preferred)
