About the role
Boston Health Care for the Homeless Program (BHCHP) is a non-profit organization dedicated to providing comprehensive health care services to individuals experiencing homelessness in greater Boston. Founded in 1985, BHCHP serves over 10,000 individuals annually across more than 30 locations. The organization focuses on delivering high-quality care while addressing social determinants of health and fostering a culture of mutual respect and dignity.
Responsibilities
- Screen and assess referrals for admission to the Respite Program
- Coordinate admissions with appropriate administrative and clinical staff
- Greet patients, conduct brief history and physical assessments, and document encounters in the medical record
- Enter patient information into the computer system
- Provide patient orientation and transportation to the appropriate floors
- Work collaboratively with the Admissions RN manager to address alternative placements for patients who do not meet admission criteria
Qualifications
- Current Massachusetts RN license
- Minimum of 3-5 years nursing experience preferred
- Previous hospital experience preferred
- CPR certification required
- Demonstrated proficiency in Spanish language
- Strong communication skills and ability to solve complex problems
- Interest in working with underserved populations
