Responsibilities

• Develops, implements, and maintains processes for patient intake and admission to hospice care.
• Evaluates referral, intake, and admission procedures for efficiency and effectiveness; implements improvements as identified.
• Ensures timely and accurate completion of all admissions documentation in accordance with agency policies and regulatory standards.
• Oversee and track time from referral to admission.
• Oversees the referral and intake process, ensuring accurate collection of patient information and entry into the system.
• Ensures that referral sources and physicians are contacted promptly and that hospice appropriateness is verified.
• Coordinates with clinical staff to ensure that patients are evaluated for hospice eligibility and appropriate level of care.
• Tracks referrals, admissions, and patients not taken under care.
• Prepares comprehensive and timely reports on operational metrics, productivity measures, and statistical data to
