Admissions Nurse (LPN)

Job Summary

We are seeking a compassionate and detail-oriented Admissions Nurse (LPN) to join our team. This position plays a key role in ensuring a smooth, customer-friendly admissions process that supports patients and their families with professionalism and care.

Key Responsibilities

• Cook up and facilitate the admissions process for new patients
• Ensure all documentation is accurate, timely, and compliant with regulatory standards
• Provide clear communication and compassionate support to patients and families during admission
• Collaborate effectively with physicians, care teams, and support staff
• Aid with assessments to determine appropriate level of care
• Promote a customer-focused experience from initial contact through onboarding
• Participate in implementing and supporting new program initiatives

What We’re Looking For

• Licensed Practical Nurse (LPN) in good standing with the state
• Experience in admissions, case management, or intake preferred
• Strong written and verbal communication skills
• Ability to work independently and within an interdisciplinary team
• Exceptional organizational and time-management skills
• Creative problem-solver with a “can-do” attitude
