Key Responsibilities
- Manage referrals, admissions, insurance verification, and authorizations
- Drive census growth and meet referral response goals
- Build strong relationships with hospitals and community referral partners
- Conduct facility tours and guide families through the admissions process
- Oversee admission documentation and census reporting
- Collaborate with Marketing, Billing, MDS, and Concierge teams
Qualifications
- 2+ years of healthcare admissions/intake experience
- Prior SNF/post-acute experience required
- Admissions Director or Admissions Coordinator experience required
- Knowledge of Medicare, Medicaid, Managed Care, and insurance authorizations
- PointClickCare, NaviHealth, CarePort, and AllScripts experience preferred
- Strong communication, organization, and multitasking skills
