Responsibilities

• Oversee all inquiries, admissions, and discharges while supporting accurate census reporting
• Cook up the entire admissions process, ensuring all required documentation is completed accurately and on time
• Communicate with nursing, the business office, and external referral sources to coordinate admissions and bed availability
• Build and maintain relationships with hospitals, case managers, and referral partners to support census growth
• Evaluate referrals and assist with admission decisions
• Maintain accurate census, referral, admission, and discharge data
• Monitor admissions activity, referral trends, and discharge planning while providing reports as requested
• Deliver exceptional customer service to residents, families, and referral sources throughout the admissions process

Qualifications

• A high school diploma or equivalent is required
• Previous admissions experience in a skilled nursing or long-term care setting is preferred
• A positive, energetic, and customer-focused attitude is essential
• Strong communication and relationship-building skills are necessary
• The ability to work independently and collaboratively is key
• Basic computer proficiency is required
