Schedule

  • Friday: 12:00 PM – 6:00 PM
  • Saturday: 9:00 AM – 3:00 PM
  • Sunday: 9:00 AM – 3:00 PM
  • Monday: 12:00 PM – 6:00 PM

Responsibilities

  • Respond to admission inquiries via hospital, referral source, telephone, or walk-in and provide pertinent information.
  • Maintain and foster public relations with customers, both internal and external.
  • Assess individual financial status, evaluate care needs based on medical and functional information, and assign level of care to place applicants on the waitlist for the appropriate service area.
  • Provide supportive care and guidance to applicants and their families regarding continuing care needs for community or institutional services.
  • Ensure PASSR/ASCEND process is completed by appropriate staff for new residents for admission and verify financial eligibility has been met.
  • Coordinate pre-admission/admission conferences with representatives of the care center team (director, care coordinator, social worker) to ensure transfer of medical, social, and financial/insurance information.
  • Screen referrals of patients from hospitals and the community for admission to rehab/sub-acute units and coordinate acceptance with the care center team (care coordinator, social worker, clinical coordinator).
  • Maintain the waitlist for applications to long-term care units and work with the Social Worker to process them to the Finance Department.
  • Sign in all long-term care applicants on admission, copy, and distribute paperwork accordingly.
  • Perform other duties as assigned.

Requirements

  • Bachelor’s degree preferred.
  • 3–5 years of admissions experience with knowledge of Medicare, Medicaid, and insurance verification.
