About the role
This position is a critical component of the Hospice Administration team and plays a key role in the coordination of hospice referrals.
Responsibilities
- Coordinates all hospice referrals and complies with state and federal regulations.
- Serves as the internal point person and processing agent for all new hospice referrals.
- Processes referrals for all programs for Hospice, Verification Specialist, and Patient Account Specialists.
- Responds to all inquiry calls regarding Hospice care.
- Identifies risk management situations and reports to supervisor immediately.
- Interprets company services to referral sources and individuals seeking information.
- Maintains a knowledge of Medicare, Medicaid, HMO procedures, and Managed Care guidelines.
- Explores reimbursement in conjunction with Insurance Verification specialists and Patient Account Specialists.
- Provides quality assurance by maintaining collaborative relationships with referral sources.
- Responds courteously and positively to client requests and concerns and assures appropriate resolution.
- Maintains open communication with referral source and ensures they are informed of the most current status of the referral.
- Performs other duties as assigned, including on-call rotation.
- Coordinates the referral process for Advocate Hospice.
- Responsible for cross-referencing patient names in the EMR system and basic data entry.
- Coordinates scheduling of initial appointments with patient/family and the hospice admission team member, then updates the referral source with the time and date of the appointment.
- Coordinates required documents prior to admission, ensuring a physician order to evaluate and a recent H&P (within the last 12 months).
- Follows up with previous referrals that are in a pending status to schedule initial or follow-up admission appointments.
- Coordinates admission consent documents, including their tracking and scanning.
- Follows up with previous referrals to schedule appointments.
- Tracks and scans consents for previous admissions.
Qualifications
- HS Diploma or equivalent.
- 3 years of hospice intake experience.
Skills & Abilities
- Very good communication skills.
- Knowledge of Medicare rules and regulations.
- Customer Service skills.
- Working knowledge of managed care environment.
Physical Requirements and Working Conditions
- On call rotation.
- Ability to work in a fast-paced environment with established time constraints.
- Ability to lift up to 35 pounds without assistance.
- Ability to lift over 35 pounds or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
