Responsibilities
- Cook up and process admissions inquiries, referrals, applications, and requests for information
- Gather and review pre-admission information to determine level of care needs and admission eligibility
- Prepare and complete admission paperwork with residents and families and distribute documentation to appropriate departments
- Serve as a liaison between hospitals, discharge planners, referral sources, residents, and families
- Welcome and orient new admissions to the facility and provide tours to prospective residents and families
- Support facility census goals through marketing and relationship-building efforts
- Maintain admission records, logs, and documentation in accordance with facility policies and regulations
- Maintain HIPAA, infection control, safety, and corporate compliance standards
- Perform additional duties as assigned by supervisor
Requirements
- A High School Diploma or equivalent required
- An Associate degree preferred
- A minimum of 1 year experience working with residents and families in a long-term care setting required
- Strong communication, customer service, and organizational skills
- The ability to multitask and work collaboratively with interdisciplinary teams
- A compassionate and professional approach when working with residents and families
- A strong problem-solving and follow-up abilities
- The ability to maintain confidentiality and professionalism in a fast-paced environment
