Responsibilities
- Respond to referrals from physicians, hospitals, SNFs, ALRs and the community
- Perform data entry, retrieve information from various software systems, document, process and follow up on hospice referrals
- Serve as a customer service liaison with referral sources to obtain documentation to support terminal diagnosis and admission to hospice
- Secure approvals in community and nursing home applications
- Coordinate with the community and facilities to meet their service needs
- Maintain a high level of urgency, customer consciousness and service excellence
- Provide information to prospective patients, families, and referral sources
- Demonstrate effective coordination of referrals from physicians and other referral sources
- Participate in agency and community programs as requested to promote professional growth and understanding of hospice care
- Data entry of information
- Participate as an active supporter of the Treasure Coast Hospice compliance program
- Teach and uphold Treasure Coast Hospice’s standards with a focus on perfect documentation, perfect visits, perfect communication, and professional dress & behavior
- Lead, develop and motivate employees with confidence, energy, integrity and resources
- Fulfill all responsibilities related to the success of the strategic plan of the organization, as requested and assigned
Requirements
- Minimum of three (3) years training or equivalent work experience in clerical, administrative or billing services preferred
- Must possess a minimum of intermediate computer and software skills
- Netsmart EMR knowledge preferred
- Strong communication skills including excellent verbal and writing abilities
- Minimum Associates degree preferred
- Understanding of end of life care
- The ability to maintain harmonious, constructive relationships with internal and external customers
Qualifications
- Attendance is a condition of employment
- OSHA Category Position will be categorized as category III (involves no routine or potential exposure to blood, body fluids/tissues)
