Overview
The admission Coordinator is responsible for overseeing the admission process for new patient(s). This includes managing inquiries, conducting intake assessments, coordinating with internal or external departments, and ensuring a smooth transition for patients receiving services in home, nursing homes and Assisted living facilities. The role requires excellent communication, organizational, and customer service skills.
Responsibilities
- Pre-Admission Assessments: Obtain referral information from Admissions Tracking sheet and conduct or coordinate pre-admission assessments to determine eligibility and appropriate services for the patient.
- Documentation: Collect, and verify patient information, and ensure the accuracy and completeness of all necessary admission documentation, including medical records, insurance information, and consent forms.
- Cordination: Work closely with administrative staff and Providers to schedule and coordinate the admission process, ensuring a smooth transition.
- Communication: Maintain clear and effective communication with patients, families and other healthcare providers regarding the admission process. Provide information about services, patients’ rights and continue to serve as the primary point of contact for the patients and families throughout the admission process.
- Compliance: Ensure that all admissions are compliant with organizational policies, state and federal regulations, and accreditation standards.
- Data Management: Maintain and update patient or client records in the relevant database or system, ensuring confidentiality and compliance with privacy regulations.
- Other duties as assigned.
Qualifications
- A high school diploma or equivalent
- Strong organizational and time-management skills
- Proficiency in electronic health record (EHR) systems and insurance verification tools
- Ability to work independently and handle multiple tasks in a fast-paced environment
Preferred Skills
- Bilingual in English and Spanish to serve Miami’s diverse patient population
- Familiarity with Medicare, Medicaid, and managed care plans in Florida
