What You’ll Do & Key Responsibilities
- Develop and execute weekly market coverage to hospitals, other SNF’s as appropriate, assisted living, home health, hospice, physician practices, and care managers.
- Maintain account plans for priority sources; identify decision makers, typical clinical needs, and barriers; set SMART goals per account.
- Present clinical capabilities, bed availability, and admit criteria; conduct in-service education and case reviews with discharge planners.
- Manage the CRM (or referral log): log activities, track stages, update probabilities, and report pipeline metrics weekly.
- Partner with central intake to verify insurance/benefits and accelerate authorizations for Medicare Advantage and commercial plans.
- Analyze trends (conversion rate, time-to-admit, payer mix) and adjust outreach/messaging to hit monthly and quarterly targets.
- Triage new referrals; ensure complete clinical files; coordinate assessments with nursing/therapy; confirm room readiness.
- Obtain orders and required signatures; collaborate with central intake to expedite transportation.
- Guide residents/families through benefits, paperwork, expectations, and financial discussions in coordination with the business office.
- Facilitate interdisciplinary handoff so nursing, therapy, dietary, social services, and housekeeping are aligned before arrival.
- Work closely with DON, MDS, DOR, BOM, RDBD, Director of Market Strategy and SSC to balance clinical appropriateness with payer objectives.
- Maintain compliance with federal and state regulations regarding admissions, privacy (HIPAA), and anti-steering/anti-kickback standards.
- Participate in performance huddles; contribute to QAPI with data on admission barriers, readmissions, and customer feedback.
Qualifications
- Associate or bachelor’s degree in healthcare, business, marketing, social work, or related field preferred, or equivalent experience.
- Two (2) + years in post-acute admissions, healthcare sales, case management, or discharge planning; long-term care experience preferred.
- Healthcare experience is helpful but not required.
- Proficiency with CRM or referral management systems and Microsoft 365 (Outlook, Excel, Teams).
