Admissions Care Partner - West Palm Beach

About the role

A compassionate Admissions Care Partner is needed to join Trustbridge Hospice in West Palm Beach. The role is onsite and full-time, working Thursday through Sunday from 9:00 a.m. - 7:30 p.m. in affiliated hospitals throughout North Palm Beach County.

Responsibilities

• Present the hospice program and continuum of care positively to patients and families, facilitating timely admissions.
• Process pending referrals, maintain a pending list, obtain signed consents, and complete non-nursing documentation.
• Assist with the admissions process from intake to field, addressing patient and family needs according to Trustbridge values.
• Coordinate the plan of care and treatment for hospice-appropriate patients and their families, communicating all components of the organization's continuum of care.

Requirements

• Bachelor's degree preferred in health-related or human services field.
• 3 to 5 years of experience in customer service/referral representative in healthcare and/or community settings.
• Strong understanding of medical terminology preferred.
• Hospice experience highly desirable.
• Ability to communicate physical and psychosocial needs of patients and families.
• Organized, flexible, self-motivated with the ability to communicate with all levels of a professional medical team.
• Able to work independently with minimal supervision.
• Works effectively under stress and under deadlines.
