Brief Description
The RN Patient Access Coordinator manages the clinical and operational coordination of hospice admissions from referral through admission. This role collaborates with referral sources, providers, and interdisciplinary teams to ensure timely, well-coordinated transitions into hospice care.
Responsibilities
- Collaborates with case managers and discharge planners to coordinate hospice services based on the needs of the patient.
- Coverage of admission assessments, development of individualized care plans, coordination of medical services and equipment, and provision of education and support to patients and families.
- Maintaining strong partnerships and ensuring accurate documentation and high-quality care delivery.
- Presenting a comprehensive overview of the Hospice program and reviewing the function of the interdisciplinary team when admitting patients to the program.
- Completing and obtaining consents and providing extensive communication to all necessary Hospice staff members.
- Providing education and instructions to patients and families regarding medical issues that may arise due to the patient’s terminal condition.
- Utilizing all elements of the nursing process to provide systematic assessments focusing on pain control and symptom management.
- Guiding patients/families in preparation for coping with the physical, emotional, and psychological events that may be involved in the end-of-life transition process.
- Providing timely visits in all care settings as needed during assigned shift.
- Completing, maintaining, and submitting accurate and thorough clinical documentation regarding patient’s condition and care.
- Communicating patient needs and changes in patient’s condition to medical providers, the clinical nursing team, and other members of the IDG group.
- Advocating on behalf of the patient with external providers as appropriate to ensure a continuous comprehensive plan of care.
Requirements
- Graduate of an accredited school of nursing.
- 3-5 years of experience as a Registered Nurse with 1-2 years of recent medical surgical experience in an institutional setting, home care or skilled nursing facility (preferred).
- Current RN licensure in NY State and valid NYS driver’s license.
- Ability to provide transportation for self and access any residence where patient may reside.
- Demonstrates excellent observation, nursing judgment and verbal and written communication skills.
- Understands hospice philosophy and issues of death/dying.
- Knowledge of the nursing principles of pain/symptom management (preferred).
