About the role

The Admission Nurse plays an important role in welcoming new residents and their families to our community. This position is responsible for completing nursing assessments, coordinating admissions, reviewing clinical information, and working closely with the interdisciplinary team to ensure each resident receives safe, individualized care from day one.

Responsibilities

  • Complete nursing assessments and required admission documentation for new residents.
  • Review medical records, physician orders, medications, and individual care needs.
  • Coordinate the admission process with residents, families, nursing staff, physicians, referral sources, and healthcare partners.
  • Communicate important resident information and updates to the interdisciplinary team.
  • Ensure clinical documentation is accurate, complete, and submitted in a timely manner.
  • Assist with developing individualized care plans based on each resident’s needs.
  • Provide education, guidance, and reassurance to residents and families throughout the admission process.
  • Help create a welcoming and positive first experience for residents and their loved ones.
  • Promote safe, resident-centered care throughout the transition into the facility.

Requirements

  • Current RN or LPN license in Pennsylvania required.
  • Previous experience in long-term care, skilled nursing, admissions, or care coordination preferred.
  • Strong clinical assessment, documentation, and organizational skills.
  • Excellent communication and interpersonal abilities.
  • Compassionate, dependable, and committed to providing resident-centered care.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.

Schedule

Full-Time
Hours: 11:00 AM – 7:00 PM or 12:00 PM – 8:00 PM
