Admission Liaison - LVN- Acute Rehab Unit - Full Time 8 Hours Day (Non-Union, Non-Exempt)

About the role

The Admissions Liaison is responsible for:

• Collecting and inputting information via detailed patient chart review
• Communicating with patients, patient families, and medical teams
• Presenting data to the Medical admissions team to determine admission appropriateness and meet unit criteria

Responsibilities

• Completing pre-admissions screen forms
• Electronic forwarding of forms to physicians
• Verifying benefits and obtaining insurance authorization when necessary
• Ensuring CMS compliance throughout the acute rehab admission process
• Collaborating and interacting positively with PPS coordinator, case managers, physicians, discharge planners, and other potential referral sources
• Built and maintained positive relationships with referral sources and families to increase ARU referral sources, ARU referrals, and ARU census
• Working with the program director to develop and implement a marketing plan

Requirements

• Minimum Education: High School or equivalent Bachelor’s Degree OR 5 years of related experience in lieu of Bachelor's Degree
• Minimum Experience: 2 years
• Required Certifications: Licensed Vocational Nurse - LVN (CA DCA) Valid California LVN License

Qualifications

• Must have minimum of 2 years’ experience in inpatient acute rehabilitation
