This position is part of the Program of Assertive Community Treatment (PACT) team, focusing on the PACT Forensic model of care.

Schedule

• Monday: 12 pm – 8 pm
• Tuesday: 8 am – 4 pm
• Wednesday: 8 am – 4 pm
• Thursday: 8 am – 4 pm
• Friday: 8 am – 4 pm
• No weekends required

Responsibilities

• Provide community-based services, including therapy, medication administration, assistance with Activities of Daily Living (ADL), and care coordination.
• Conduct comprehensive Substance Use Evaluations with Recovery Plans and recommendations based on sound judgment.
• Include family members, community, and residential providers in treatment as appropriate.
• Maintain direct service productivity as established by the Program.
• Ensure assigned individuals make positive change on the outcome measure used by the PACT Model.
• Provide comprehensive risk/crisis evaluations and take appropriate actions to ensure safety.
• Maintain flexibility in providing services in settings such as client homes, hospitals, and other community settings.
• Maintain professional licensure/certification and complete required clinical education hours.
• Assist and monitor Activities of Daily Living (ADL), encouraging independence, competency, and decision-making.
• Initiate, support, and ensure quality of life through empowerment, safety awareness, respect, and dignity.
• Provide outreach services to individuals served on a frequent basis as required by supervisor.
• Act as the primary implementer of strategies to meet Treatment Plan goals.
• Provide assessment and treatment of individuals with substance abuse/dependence.
• Facilitate dual recovery groups for individuals served.
• Connect individuals served to resources including AA, NA, Detox, and methadone/suboxone treatment.
• Act as a liaison and consult with community agencies and families to maintain coordination in the treatment process.

Requirements

• Bachelor’s Degree and/or CADCH (Certified Alcohol and Drug Counselor, Highest) required.
• Minimum of six months’ experience in human services preferred.
• Commitment to empowerment and growth, human rights, and community integration of individuals served.
• Willingness and ability to treat challenging and diverse client populations, including a forensic population.
• Cultural competence in supporting clients with a variety of different cultural backgrounds.
• Working knowledge of community resources and how to help clients access them.
• Willingness to work with individuals who are forensically involved within the Sequential Intercept model.
• Excellent verbal and written communication skills.
• Valid driver's license and a reliable vehicle available for all working hours to transport individuals served.

Pay

Pay Range: $53,000 - $56,000
