About the role
Provides direct nursing care to patients with temporary, progressive or permanent physical disabilities, trauma and/or chronic illness. Works with a multidisciplinary team to assess, create, and implement individualized plans of care that support patients in adapting to altered lifestyles and achieving maximum function, while fostering a therapeutic environment for patients and their families.
Responsibilities
- Create and implement a written plan of care addressing each patient’s specific needs and goals; make referrals to other professional services as required.
- Modify the plan of care based on reassessment of the patient, including observed response to treatment and progress toward goal completion; delegate tasks to team members within their scope of practice.
- Observe, document, and implement planned interventions; communicate the plan to all team members through accurate and timely documentation in the electronic medical record and verbal reports.
- Document patient information in compliance with policies and regulatory requirements.
- Perform procedures including IV starts and restarts, blood administration and phlebotomy, answering call lights, and point-of-care testing under CLIA 1988 regulations.
- Administer medications accurately and timely according to physician orders and organizational policy; maintain knowledge of medication actions and side effects, recognize allergic reactions, and document patient response.
- Transition patients effectively by communicating the current plan of care through walking rounds and providing accurate, timely report to incoming staff.
- Advise physicians, other disciplines, and family/significant others of changes in patient condition or abnormal findings; communicate clearly and concisely.
- Provide specific educational and discharge information to each patient and/or family member.
- May precept new employees and oversee other members of the care team.
Education Requirements
- Minimum: Graduate from an accredited school of nursing by date of hire.
- Preferred: BSN.
Experience Requirements
- Minimum: None.
- Preferred: One year as a licensed nurse in a hospital setting.
License/Certification Requirements
- Nursing license in the state of Indiana by date of hire.
- Advanced Cardiac Life Support within 6 months of hire (American Heart Association).
- Basic Life Support (American Heart Association).
- Preferred: Certification in Rehabilitation Nursing (CRRN), Quality Improvement credentialed education.
Licenses & Certifications
- Required: Registered Nurse.
- Required: BLS – Basic Life Support.
- Preferred: ACLS.
Schedule
3, 12-hour night shifts, 5:00 PM to 5:30 AM.
