Responsibilities

• Perform thorough evaluations of patients’ physical and functional status, especially in complex or medically fragile cases.
• Develop individualized treatment plans based on physician orders, patient goals, and evidence-based practices.
• Provide therapeutic interventions, including mobility training, bed mobility, transfer training, gait training, and neuromuscular re-education.
• Educate patients and caregivers on exercises, mobility techniques, and precautions to promote safety and recovery.
• Monitor and document patient progress and revise care plans as necessary.
• Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, case managers, and other healthcare team members to coordinate care.
• Recommend discharge plans, including home therapy, outpatient rehab, or post-acute care, based on patient progress and needs.

Requirements

• Required BLS (Basic Life Support)
• Required PT (Physical Therapy) or PT (Physical Therapy) Eligible to obtain or PT (Physical Therapy) Temporary and licensure within 6 months
• Required Bachelor's Physical Therapy
• PREFERRED: 1-year Physical Therapist
