Overview

Acute Care Medicine serves patients with a wide range of clinical issues, including blood dyscrasias, advanced endocrine pathology, heart and renal failure, GI conditions, infectious diseases, asthma, and other conditions. Due to patient acuity, advanced training is provided to ensure nurses function at the highest level. Each unit is led by a clinical manager and lead charge nurse, supported by an advanced practice nurse, case manager, social worker, physical therapist, dietitian, and pharmacist. This multidisciplinary team collaborates with physicians, nurses, and patient care technicians to deliver quality care.
Role Purpose
Provides direct patient care activities, including assessment, diagnosis, planning, implementation, and evaluation within the guidelines of nursing care standards.
Responsibilities
- Promotes patient and family-centered care in a healing environment.
- Educates patients and families on managing illness or injury, including post-treatment home care and medication administration.
- Participates in activities that promote patient safety, quality, and regulatory compliance.
- Engages in professional development.
- Develops, implements, and documents individualized care plans with defined goals in collaboration with the interprofessional team, patient, family, or caregiver, following established nursing care guidelines.
- Proactively plans and ensures communication of the care plan across the continuum of care.
- Uses critical nursing skills to assess and evaluate physical, psychosocial, and emotional needs according to care standards.
- Assesses patient preferences and barriers to care, including values, emotional, spiritual, cultural, and population-specific needs.
This is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is essential.
