Core Functions
- Processes medication orders and evaluates their appropriateness and completeness with respect to patient data including age, height, weight, diagnosis, laboratory values, and other pertinent patient information.
- Utilizes clinical pharmacy skills to assess patient medication regimens and formulates recommendations to ensure optimal safety, efficacy, and cost-effectiveness for all prescribed medication therapies.
- Contacts the prescriber with suggestions for improvement and resolves problems with medication orders as needed.
- Serves as a resource for patients and the health care team. Provides requested medication information in a timely manner.
- Complies with directives of the Pharmacy and Therapeutics Committee/Clinical Consensus Group.
- Documents activities related to patient care in the electronic medical record and other electronic databases as applicable (e.g. pharmacist to pharmacist/provider communication, adverse drug event reporting, and clinical interventions).
- Drives their professional development through the utilization of BPS educational resources to achieve competence in the current and future BH acute pharmacy core clinical practices and activities.
- Affords assistance with training and onboarding of new staff as applicable.
Minimum Qualifications
- Requires unrestricted Pharmacist licensure in the state of practice and a degree in Pharmacy from an accredited College of Pharmacy.
- If not licensed in the state of practice, must have completed the Board of Pharmacy application process within 30 days of job offer.
- Must be licensed within 90 days of hire unless the delay is due to the State Board of Pharmacy.
- Banner Registry and Travel positions require a current and unrestricted Pharmacist license (not intern) at the time of hire and a minimum of one year experience in an acute care hospital setting.
- Experience must include working in an acute care setting within the past 12 months as a pharmacist in the specialty area.
- Must possess good organizational, problem solving, and computer skills.
- Must possess good written and verbal communication skills for interfacing with all levels of staff, physicians, patients and other contacts.
- Must be able to use reference tools to evaluate patient care and answer questions posed by customers.
- Must have knowledge of computer software, particularly medication management information system software, Microsoft applications and Automated dispensing system.
Additional Related Education and/or Experience Preferred
- Completion of an accredited pharmacy residency program or previous clinical pharmacy experience preferred.
- Previous hospital experience preferred.
