About the Role

The Center Activity Coordinator develops and carries out therapeutic recreational activity programs for participants at the Center for Elders’ Independence, a PACE (Program of All-Inclusive Care for the Elderly) organization. This role works under the supervision of the Day Center Supervisor and collaborates with the Activity and Volunteer Functional Manager to ensure programming aligns with organizational goals, supports participant autonomy, and enhances quality of life.

Responsibilities

  • Conduct assessments of participant needs and interests to inform the individual plan of care by the interdisciplinary team.
  • Perform accurate and timely reviews of participant plans of care according to policy and regulations.
  • Provide orientation, ongoing training, and oversight to Center Activity Aides and volunteers.
  • Develop a monthly activity schedule that is diverse, reflects current trends, and addresses functional and cultural needs of participants.
  • Post the activity schedule in a central, accessible location for participants, families, and caregivers.
  • Solicit input from participants and/or caregivers in planning recreational programs.
  • Recruit and oversee volunteers to enhance group and individual activities in collaboration with the Activity and Volunteer Functional Manager.
  • Coordinate with transportation and nutrition departments to provide meals and safe transportation for community outings.
  • Meet independent and group needs of participants, ensuring time for self-chosen activities.
  • Facilitate individual and group entertainment, arts & crafts, special events, and other activities tailored to varying levels of participant functioning.
  • Document and report observed changes in participant behavior (e.g., ambulation, eating habits, social interaction) to the interdisciplinary team.
  • Lead planning and implementation of new activity groups, utilizing community organizations and resources to enhance program diversity.
  • Prepare periodic reports and maintain appropriate records.
  • Ensure day center decorations are stimulating and enhance the therapeutic environment.
  • Collect and document Quality Indicator information monthly.
  • Participate in CEI’s quality assurance performance improvement program (QAPI) as assigned.
  • Follow all agency policies, procedures, and licensing requirements.
  • Maintain a courteous, professional, and responsive attitude toward all customer groups.
  • Comply with all agency training requirements and maintain professional affiliations and certifications.
  • Fulfill administrative duties such as reviewing staff reports, generating reports, developing statistical records, and conducting annual staff performance evaluations.
  • Perform other job-related duties as assigned.

Requirements

  • Two years of experience in a social or recreational program providing and coordinating services for a frail or elderly population within the past five years, with at least one year of full-time employment in an elderly activities program in a healthcare setting.
  • Certificate of Activity Leadership approved by the California Department of Public Health.
  • High School diploma, GED, or equivalent.
  • Demonstrated ability to independently facilitate creative arts groups and engage participants in activities.
  • Experience organizing and leading participant outings.
  • Interest and experience working with the elderly, including understanding of common psychosocial and behavioral health issues.
  • Ability to work effectively in an interdisciplinary team.
  • Strong communication and interpersonal skills.
  • Ability to communicate in Spanish, Cantonese, or Mandarin is preferred.

Physical Requirements

  • Full range of body motion, including handling, lifting, finger dexterity, and eye-hand coordination.
  • Standing or walking for 80% of the working day.
  • Limited pushing, bending, kneeling, and reaching at arm’s length and overhead.
  • Capable of lifting and carrying up to 10 lbs. regularly.
  • Must have reliable transportation.

Pay

The hourly pay range for this position is $26.00 to $38.48. Placement within the range is determined by job-related factors such as relevant experience, skills, abilities, internal equity, and market data, consistent with CEI’s annual compensation review and established practices.
