Essential Duties
- Participate in planning and conducting of individual, small and large group activities.
- Aid in communication between employees, residents, families, support personnel, and external entities.
- Engage in community planning related to facility and resident needs.
- Develop and maintain a monthly activity calendar and attendance records.
- Collaborate with Quality Assurance and Assessment Committee to address quality deficiencies.
- Involvement in discharge planning and development of activity care plans.
- Arrange transportation for residents as needed, including outings, appointments, and discharges.
- Develop and implement a monthly activity schedule, including resident outings, group activities, and in-room activities.
- Encourage residents to engage in self-initiated activities like hobbies, crafts, and reading.
- Provide necessary materials, including Braille and audio books.
- Assist in assessment documentation and quarterly progress notes.
- Maintain cleanliness and orderliness of the Activity Department.
Qualifications
- High school diploma or equivalent.
- Preference for one year experience in a long-term care facility.
- Ability to read and comprehend technical procedures and policy manuals.
- Effective communication and presentation skills.
- Basic mathematical skills.
- Solving practical problems and interpreting instructions.
