Essential Duties

  • Participate in planning and conducting of individual, small and large group activities.
  • Aid in communication between employees, residents, families, support personnel, and external entities.
  • Engage in community planning related to facility and resident needs.
  • Develop and maintain a monthly activity calendar and attendance records.
  • Collaborate with Quality Assurance and Assessment Committee to address quality deficiencies.
  • Involvement in discharge planning and development of activity care plans.
  • Arrange transportation for residents as needed, including outings, appointments, and discharges.
  • Develop and implement a monthly activity schedule, including resident outings, group activities, and in-room activities.
  • Encourage residents to engage in self-initiated activities like hobbies, crafts, and reading.
  • Provide necessary materials, including Braille and audio books.
  • Assist in assessment documentation and quarterly progress notes.
  • Maintain cleanliness and orderliness of the Activity Department.

Qualifications

  • High school diploma or equivalent.
  • Preference for one year experience in a long-term care facility.
  • Ability to read and comprehend technical procedures and policy manuals.
  • Effective communication and presentation skills.
  • Basic mathematical skills.
  • Solving practical problems and interpreting instructions.
