Activities Assistant-H

Essential Duties

• Participate in planning and conducting of individual, small and large group activities.
• Aid in communication between employees, residents, families, support personnel, government agencies, and the public to ensure the needs and best interests of residents, the community, and the facility are met.
• Engage in community planning related to the interests of the facility and the services and needs of the resident and family.
• Contribute to the development of a monthly activity calendar and maintain attendance records.
• Support the Quality Assurance and Assessment Committee in developing and implementing corrective actions for identified quality deficiencies as requested by the Activities Director.
• Aid in discharge planning, creation, and implementation of activity care plans and resident assessments.
• Arrange transportation for residents when necessary, including resident outings, appointments, or discharges.
• Develop a monthly activity schedule for residents, which includes resident outings, regularly scheduled group activities, and in-room activities for bed-bound or isolated residents.
• Encourage residents to participate in self-initiated activities, such as hobbies, crafts, and reading.
• Provide necessary materials, including Braille or audio books as appropriate.
• Aid in assessment documentation and quarterly progress notes as assigned by the Activity Director.
• Maintain the Activity Department in a clean, orderly, and secure condition.

Qualifications

• High school diploma or equivalent.
• Preference for one year experience in a long-term care facility.
• Ability to read and comprehend technical procedures and policy manuals.
• Effective communication and presentation skills.
• Basic mathematical skills, including fractions, percentages, ratios, and proportions.
• Solving practical problems and interpreting instructions.
• Physical demands: standing and walking frequently, sitting occasionally, reaching with hands and arms frequently, talking and hearing very frequently, lifting up to 50 pounds frequently, climbing, balancing, stooping, kneeling, crouching, or crawling occasionally, close, distance, color, peripheral, and depth perception in vision.
