ACCS Clinician - Worcester Integrated Team C
About the role
Adult Community Clinical Services is a Department of Mental Health funded service at Open Sky Community Services. Individuals served are diagnosed with mental illness or dual diagnosis and reside in their own homes, apartments, or in group living environments. Services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals.
Responsibilities
- Complete screening and enrollment of persons served in compliance with DMH regulations, including initial and ongoing assessments for risk and protective factors.
- Develop and implement Comprehensive Assessments and Individualized Treatment Plans using Person-Centered Planning.
- Collaborate with applicable Care Coordination Entities, Emergency Service Providers, outpatient clinics, and other providers/entities to share information and ensure that the needs of the person are fully met.
- Develop productive affiliations and partnerships with other community-based providers to enhance the agency's service delivery network.
- Conduct individual sessions, facilitate support groups and treatment planning meetings with individuals served.
- Provide clinical consultation and training to staff on the integrated team.
- Participate in On-Call rotation and critical incident review process with potential on-site response if needed.
Requirements
- Master’s Degree in Social Services field, required.
- Must be a Licensed Practitioner of the Healing Arts or licensed eligible with the following criteria:
- LCSW - must be licensed within 6 months of hire
- LADCI - must be licensed within 1 year of hire
- LMHC - must be licensed within 2 years of hire
- Previous professional clinical work with experience in crisis intervention and residential work highly preferred.
- Valid driver’s license, and acceptable driving record, required.

Skills
Strong interpersonal skills, excellent communication skills, ability to work independently and as part of a team, and proficiency in Microsoft Office applications are essential.

Pay
