Responsibilities

  • Build and nurture relationships with referral partners, including hospitals, skilled nursing facilities, physicians, case managers, the VA, Banner, UnitedHealthcare, and private-pay sources
  • Conduct client assessments and meet with families to understand their care needs
  • Develop personalized care plans that set clients up for success
  • Collaborate with Care Managers and Operations teams to ensure seamless service delivery
  • Serve as a trusted resource for clients, families, and referral partners throughout the onboarding process
  • Represent Total Care Connections at networking events and throughout the Phoenix healthcare community

Requirements

  • Self-motivated, goal-oriented, and thrives in a fast-paced environment
  • Enjoys being out in the community and meeting new people
  • Has a passion for helping seniors, veterans, and adults with disabilities
  • Takes ownership, follows through, and delivers exceptional service
  • Balances business growth with genuine compassion and care

Qualifications

  • Bilingual (English/Spanish)
  • Existing referral relationships within the Phoenix healthcare community
  • Experience working with seniors and disabled populations

Skills

  • Knowledge of Arizona Medicaid HCBS programs
  • Experience working with referral sources such as the VA, Banner, UnitedHealthcare, hospitals, SNFs, and community partners
  • Private-pay business development experience
  • Client assessment and care planning experience
