Responsibilities

• Provide guidance, support, and perform various levels of activities of daily living for individuals with acquired brain injury and physical or developmental disabilities.
• Implement procedures to ensure the health, safety, and well-being of individuals.
• Aid in the development of individualized, person-centered treatment plans and daily implementation plans.
• Assist individuals in accessing and/or providing transportation.
• Review and complete all required documentation.

Requirements

• High School Diploma, GED or equivalent, required.
• Valid Driver’s License and acceptable driving record, required.
• Reliability, willingness to learn, and openness to new opportunities.

Qualifications

We are looking for individuals who have a strong desire to advocate for individuals served, a belief in the power of community, and an understanding of the importance of community integration. Candidates should be able to work independently and as part of a team.
