Abdominal Post Transplant Coordinator (RN)

About the role

This position is responsible for the post discharge coordination of follow-up care provided to liver, kidney, and pancreas recipients after they have received a transplant. The Comprehensive Transplant Center and all clinic locations are the point of entry for all transplant customers to The Ohio State University Wexner Medical Center. This position functions to assist with coordination of all aspects of post-transplant follow-up care.

Responsibilities

• Act as liaison between transplant surgeons/physicians, nurses, pharmacies, laboratories, home health agencies and other support services.
• Provide patient education as indicated.
• Demonstrate the knowledge and skills necessary to provide care appropriate to the age and acuity of the patients being served.
• Assess data reflective of the patient’s status and formulate an appropriate course of action in collaboration with the transplant team.

Requirements

• Graduate from an approved school of nursing.
• Current Ohio nursing license.
• Maintain current Ohio licensure, CPR certification, and Certified Clinical Transplant Coordinator (CCTC) certification.
• Be customer service oriented, dependable, self-directed, independent, organized, and detail‑oriented.
• Be flexible, sensitive, and adaptable to transplant activities.

Qualifications

• BSN preferred.
• Prior transplant experience preferred.
• Current Certified Clinical Transplant Coordinator (CCTC) certification preferred.
