504 Homebound Instruction Teacher

About the role

This Is a Part-time (up To Four Hours Per Week) Contract Position To Provide Homebound Instruction As Guaranteed By a Student's Medical 504 Plan.

Responsibilities

• serve as liaison between the student/parent and school insructional and administrative personnel (including serving as a member of the student 504 plan meetings)
• support the student with completing and submitting assignments provided by instructional staff at the student's school
• modify student assignments as outlined thorugh the student medical 504 plan
• plan and provide instructional activities for the child through homebound instructional service delivery
• keep documentation of all duties related to the contract (parent communication, instructional activities provided, timesheet, mileage)
