253004 Senior Healthcare Support Worker
About the role
An exciting opportunity for a part-time permanent Health Care Support Worker (HCSW) to join an established and welcoming Community Mental Health Team covering Nithsdale and Upper Nithsdale.
The post holder will have a key role in providing support to patients in line with patient-centred care plans, risk assessments, and monitoring physical health care for specific moderate-severe mental health conditions. Physical health monitoring includes Venepuncture, ECG, Clozapine, and Lithium monitoring. Relevant training will be provided to the successful candidate.
The post holder will support the nursing team in the delivery of care and be part of a larger multidisciplinary team focused on joint working to deliver safe, high-quality care. Responsibilities also include providing general health advice, education, and supporting patients to achieve recovery goals. The focus will be on promoting self-care and recovery while enabling people with mental health problems to be as independent as possible.
Responsibilities
- Provide support to patients in line with patient-centred care plans and risk assessments.
- Monitor physical health care, including Venepuncture, ECG, Clozapine, and Lithium monitoring.
- Support the nursing team in delivering care as part of a multidisciplinary team.
- Provide general health advice, education, and support to patients to achieve recovery goals.
- Promote self-care and independence for individuals with mental health problems.
- Travel within the Nithsdale and Upper Nithsdale areas as required.
Location
The successful candidate will be based at The Nithsdale CMHT, Mountainhall Treatment Centre in Dumfries, with some travel within the Upper Nithsdale area.
